This Personal Data Protection Notice (“Notice”) is prepared in accordance with the Personal Data Protection Act 2012, Republic Act No. 10173
AstraZeneca Pharmaceuticals (Phils.), Inc. (“AZ”) respects the privacy of your personal date and will take all steps reasonable to keep your personal data secure. It is important that you first take the time to read through and understand this Notice. After you are satisfied that you understand the information in this Notice, you must sign this consent form.
1. AZ, as your partner in healthcare for patients, is operating with tools and systems that already, or which may require the collection and processing of your personal data. Your personal data may consist of your name, e-mail address, clinic or hospital address, Philippine Regulatory Commission ID No. (PRC No.), telephone and mobile numbers, and any other related information.
2. Any personal data supplied by you or collected from you, directly or indirectly, will be used for the following (“Purposes”) only:
a. because it is necessary to enable us to give you medical and scientific information about our products, or to enter into transactions with you or to deliver the necessary tools, services and/or communication in connection with our products and such transactions;
b. to update and enhance our records;
c. protecting and enforcing our mutual rights and obligations;
d. compliance with any applicable rules, laws and regulations, codes of practice or guidelines; and/or
e. any other purposes as may be related to the foregoing.
3. You acknowledge and consent to your personal data being transferred or disclosed to the following entities, whether located in Philippines or overseas:
a. our licensed agents and/or business alliance partners;
b. governmental departments and/or agencies, regulatory and/or statutory bodies;
c. our third party service providers, including those who process and provide payment, administrative or other services in connection with the operation of our business, or data processors
4. The provision of your personal information is voluntary. However, if you do not provide us with the personal information requested, we may not be able to provide you with information about our products, including medical and scientific information and exchanges, as well as invitations to meetings where we give information about our products, and other opportunities to continue to partner with you.
